At first, we had trouble finding a free weekend. There was volleyball, soccer, Girl Scouts, and school activities. Once we made it through the sports? seasons, we had a little more time on our hands. I thought for sure we?d be able to tackle the project in just a few hours. Yeah, I?m hilarious.

On our first planned work weekend, the tween was diagnosed with scarlet fever (no, seriously) so the weekend was spent hauling her to the doctor, then the urgent care center, and then to the pharmacy. We spent the rest of the weekend nursing her back to health and keeping the children separated.

I can?t even remember what happened the next weekend, but I know the work day disappeared before I even knew it. Finally, I just started ripping out the carpet. I thought I could at least see what it would take to make it happen. The next thing I knew, I had ripped all the carpet out and was pretty much locked into the project.

That was when I realized the stairs needed a little trim work before they could be finished. There were a few gaps here and there, due to the builders not taking the time to finish them properly. After all, they probably never imagined an insane woman ripping out the carpet one random Saturday night.

By that point, Home Depot was closed so our project was on hold. And Sunday was already booked. The rest of the week was fall break, with family fun activities and kids underfoot all day. Today was supposed to be another work day, but I hadn?t considered the evening?s plans with this idea. The girls are having friends spend the night, which makes dealing with wet paint a little difficult.

Okay, so tomorrow, right? Except by the time all our visitors are home, we still have the problem of finishing the job and getting the paint dried before the girls need to go up stairs for bed. And, this is a problem every weekend ? the girls are around, the pets are in the way. How do we paint stairs in a busy house?

I think we?ll have to get started early one morning, right after the girls are at school. Even if the stairs are not dried before they get home, at least we?ll have a few more hours to bedtime. Now, who wants to keep 2 cats and a dog while we paint?

It is time to put the nature versus nurture debate to rest and embrace growing evidence that it is the interaction between biology and environment in early life that influences human development, according to a series of studies recently published in a special edition of the Proceedings of the National Academy of Sciences (PNAS).

"Biologists used to think that our differences are pre-programmed in our genes, while psychologists argued that babies are born with a blank slate and their experience writes on it to shape them into the adults they become. Instead, the important question to be asking is, 'How is our experience in early life getting embedded in our biology?'" says University of Toronto behavioural geneticist Marla Sokolowski. She is co-editor of the PNAS special edition titled "Biological Embedding of Early Social Adversity: From Fruit Flies to Kindergarteners" along with professors Tom Boyce (University of British Columbia) and Gene Robinson (University of Illinois).

Sokolowski, who is a University Professor in the Department of Ecology & Evolutionary Biology (EEB), the inaugural academic director of Uof T's Fraser Mustard Institute for Human Development and co-director of the Experience-based Brain and Biological Development Program (EBBD) at the Canadian Institute for Advanced Research (CIFAR) says that relatively little is known about the gene-environment interplay that underlies the impact of early life adversity on adult health and behaviour.

In one of the studies in the series, Sokolowski and her colleagues found that chronic food deprivation and lack of adequate nutrition in the early life of the fruit fly Drosophila melanogaster had significant impact on adult behaviour and quality of life. Fruit flies are especially useful for genetic studies because they share a surprising number of qualities with humans, are inexpensive to care for and reproduce rapidly, allowing for several generations to be studied in just a few months.

The researchers examined two types of fruit flies with variants in the foraging gene (for) known as rovers and sitters because of their different behaviours in the presence of food.

When well fed as larvae, rover adults exhibit darting exploration into open areas as they move about in search of food, while sitters show little of this behaviour. When nutritionally deprived as larvae, both rover and sitter adults exhibit darting exploration. Further, the sitters that faced nutritional adversity in early life displayed a reduction in their ability to reproduce. Rovers exhibited no effect on their reproductive fitness.

"The foraging gene makes an enzyme called PKG, which is found in the fly as well as in most other organisms, including humans. When faced with a nutritionally adverse environment while growing up, the levels of the enzyme dropped in flies," says Sokolowski. "This told us that the foraging gene listens to its environment." Transgenic manipulations of PKG levels altered darting exploration in well fed but not nutritionally deprived flies.

The research team included James Burns, a CIFAR junior fellow in Sokolowski's lab, U of T EEB professor Locke Rowe and EEB post-doctoral fellow Nicolas Svetec, as well as colleagues from the Universitiy of British Columbia and the Universit? Paris-Sud. The findings are reported in the paper "Chronic food deprivation in early life affects adult exploratory and fitness traits, in the October 16, 2012 issue of the Proceedings of the Nataional Academy of Science.

The papers in the volume are authored largely by CIFAR researchers, and comprise a multidisciplinary collection of research into fields from molecular genetics, evolutionary biology and neuroscience, to social and behavioural science, epidemiology and social policy ? as well as the emerging field of epigenetics, which investigates deviations in a gene's ability to produce its products (e.g. RNA, protein) caused by mechanisms other than changes in an organism's underlying DNA sequence.

The collection of papers in the volume sets out an emerging new field of the developmental science of childhood adversity, and changes conventional understanding of the early years of human life.

"This is the first volume of collected research to provide a substantial and comprehensive picture of the interaction between experience and biology in the early years," says Sokolowski.

"Developmental neuroscience is extraordinarily intricate and complex, and so by approaching this question from multiple angles we're able to reveal a convergence on a number of themes and set a clearer direction for future research."

Roger B. of Pennsylvania says, ?Hey, how about the CBO or some neutral scoring facility?? Thank you Roger, your response is thoughtful. The key is to go back and look at CBO scoring techniques. Turn the clock back five years to see how well they have done. Their margin of error is measured in the hundreds of billions on either side and in either direction. They are constrained by a set of rules passed by Congress, who wrote the rules to make themselves look good. Sadly, this is now a flawed system.

Media responses were appreciated as well. Some asked why there is such a difference between the analyses. Has Romney been completely clear? What are the substantive issues that are unresolved? These are great questions, which demand complex answers although both candidates do not give them. We will offer some thoughts in the closing comments.

No one talks much about expectation changes or behavioral changes. The tax debate ignores the difference between static versus dynamic analysis. It is not a simple statistical matter.

However, the fundamental issue is simple. If you want more of something, lower the taxes on it. If you want less of something, raise the taxes on it. I learned that from a fine economics professor and good friend, Bill Poole. Apply that concept first; it makes compromise easier to obtain if you do. If only our politicians would listen! That principle includes personal income and aggregate GDP.

Here is where we stand today. We talk about a fiscal cliff in a conversation laden with political acrimony but without solutions. We talk about tax changes, yet the most important tax change, a 2% cut in the payroll tax that would deliver a thousand dollars in spending power to every household in America and relieve working people of the most regressive taxation, is not on the table. Neither Obama nor Romney has proposed extending the payroll tax cut. They are both busy defending the respective special interests that are on their sides of the political aisle.

Why not put the payroll tax cut on the table? Why not debate regressive taxes and whether to make them permanent? The reason is simple. The so-called political forces of the United States (Democrats and Republicans), aspirants for the White House, and incumbent congressmen all have the same issue. They do not want to give the revenue back to the people who work and pay taxes in the US. They have their own program agendas that they want to push forward. The common issue of tax relief is talked about and not acted upon.

Taxes are a big deal in the campaign. Are we drilling deep enough in the debate? No. Do we have to resort to looking at analyses that come from private organizations whether it is the Tax Foundation, the Tax Policy Center, or somewhere else? Yes. That is the best we have.

Let me add one final comment on the tax-comparison pieces. The analysis of taxation was done by the Tax Foundation, not us. We provided readers with the link, as we have also done with the Tax Policy Center. Those folks are responsible for their numbers.?

Cumberland assembled the analysis of municipal taxation (taxation of tax-free bonds or tax treatment of tax-free bonds) ? whether they are going to change and what the rates will be. We work in that turf.

Our conclusions are clear. The benefits of tax-free bonds are huge. The political constituency that supports them consists of school boards, sewage authorities, airport authorities, counties, townships, and states. That is a massive constituency. Every finance officer in those agencies knows that the cost of finance goes up if you eliminate the tax exemption of interest or the favorable tax treatment of interest on tax-free bonds. Simply put, there is almost four trillion dollars of outstanding state and local government debt in the US. It is turning over, rolling over, refunding, or otherwise changing every day.

If you add 100 to 150 basis points to the cost of the financing, that added cost falls on state and local government issuers. Unlike the federal government, those organizations must balance their budgets. Thus they will find the revenue to pay the higher debt service from one source or another. They will raise user fees, raise local taxes, hike property taxes, and/or increase sales taxes. One way or another, they will pass on the cost added by increased debt service. Think about it. Do we really want to burden state and local governments in the US at the time when they are retrenching, deleveraging, and altering the entire concept of provision of their services? That, too, is a question that should be part of the political debate this year.

My colleague Michael McNiven makes an interesting point of comparison between Obama and Romney. Mike uses the state and local government balancing requirement to illustrate his view. Michael wrote:

?Romney has not been completely clear. A lot of what he proposes is conceptual and vague. He says he will balance the budget by 2020, but is not clear on how he will do so. What we do know about him, however, is that he has balanced every budget within his responsibility for the balance of his career. This includes business as well as public budgets. He does bring credibility and a transparent track record to this budget balancing issue. Yet, how and when he would balance this federal budget is an open question. He has indicated his strong desire to do so, which is consistent with his personal record.

Obama has been clear on the deficit: he has run sequential trillion-dollar deficits. With Obama?s future plans, we will have deficits for a very long time. All of the improvements take place in out years and are speculative as to results. Using history as a clue, there is no historical record of Obama ever balancing a budget."

Archer fish hunt insects with water jet six times stronger than their muscular power

ScienceDaily (Oct. 24, 2012) ? Archer fish knock their insect prey out of overhanging vegetation with a jet of water several times more powerful than the fish's muscles. New research now shows that the fish generate this power externally using water dynamics rather than with any specialized internal organs.

The research, published Oct. 24 in the open access journal PLOS ONE by Alberto Vailati and colleagues from the University of Milan, provides the first explanation for how archer fish can generate such powerful jets to capture their prey.

Other animals like chameleons and salamanders store energy in collagen fibers in their bodies and abruptly release this stored energy to project their tongues at high speeds, but previous studies on archer fish have ruled out such specialized organs as the source of the powerful water jets.

The authors of the current study used high-speed video recordings of archer fish knocking insects out of overhanging plants to study the mechanics of the water jet and found that the fish could modulate the velocity of the jet to create a single large water drop that hit the prey with a large force. According to the authors, this approach avoids the evolutionary costs needed to develop special internal structures to store these large amounts of energy.

"The origin of the effectiveness of the jet squirted by archer fish has been searched for inside of the fish for nearly 250 years. The striking finding of our work is that a large amplification of muscular power occurs outside of the fish and leads to a very powerful impact of the jet with the prey," says Vailati.

Obama offers a glimpse of his second-term priorities

DAVENPORT, Iowa (Reuters) - President Barack Obama predicted passage of immigration reform and a deficit-reduction deal, offering a fresh glimpse of his second-term agenda as he fights for votes in the final stretch of the tight race for the White House.

In a newspaper interview released on Wednesday ahead of an eight-state campaign blitz that began in Iowa, Obama also suggested Republicans were bolstering his re-election effort by alienating Hispanics ahead of the November 6 vote.

He told the Des Moines Register he was confident that comprehensive immigration reform would be approved next year and predicted he would strike a deal with Republicans in the U.S. Congress within six months to reduce the budget deficit.

He made the comments in an interview with the newspaper's editors that was originally conducted off the record. After the newspaper complained about the restriction, the White House released a transcript.

"Since this is off the record, I will just be very blunt. Should I win a second term, a big reason I will win a second term is because the Republican nominee and the Republican Party have so alienated the fastest-growing demographic group in the country, the Latino community," Obama said in the interview.

Republican challenger Mitt Romney has joined many in his party in taking a tough approach to illegal immigration, a stance that has helped Obama open a substantial lead in polls among Hispanics.

The growing electoral clout of Hispanics, who now comprise 16 percent of the U.S. population, could make a difference in election battleground states like Nevada, Colorado, Florida, Virginia and Ohio.

Two weeks before the election, Obama and Romney are locked in a close battle and are competing furiously for key voting blocs like Hispanics and women.

The effort to win women voters also came to the forefront on Wednesday with a controversy over comments about rape by Indiana Republican Senate candidate Richard Mourdock.

Obama's campaign moved quickly to link Romney with Mourdock, who said in a debate with Democrat Joe Donnelly on Tuesday that pregnancy caused by rape is "something God intended to happen."

Obama believed the comment was "outrageous and demeaning to women," said campaign spokeswoman Jen Psaki, adding that she was perplexed Romney would not demand an ad he filmed in support of Mourdock be taken off the air.

'A REMINDER' ON WOMEN'S HEALTHCARE

"This is a reminder that a Republican Congress working with a Republican president, Mitt Romney, would feel that women should not be able to make choices about their own healthcare," Psaki told reporters on the flight to Iowa.

Obama has criticized Romney for his opposition to abortion rights except in cases of rape, incest or the health of the mother. Romney's campaign tried to distance him from Mourdock's remark, but did not demand that the ad be pulled.

"Governor Romney disagrees with Richard Mourdock, and Mr. Mourdock's comments do not reflect Governor Romney's views," Romney campaign spokeswoman Andrea Saul said. "We disagree on the policy regarding exceptions for rape and incest, but still support him."

The Indiana controversy was reminiscent of the uproar over Missouri Republican Senate candidate Todd Akin's comments earlier this year that women's bodies have defenses against pregnancy after "legitimate rape."

Polls show a deadlocked race nationally. A Reuters/Ipsos online tracking poll gave Romney a 1-point edge on Wednesday, 47 percent to Obama's 46 percent.

With the race so close, both candidates were stepping up their campaign schedules. Obama plans to visit eight states in a two-day marathon and will sleep on Air Force One on Wednesday night. Romney will hit Nevada and Iowa on Wednesday before spending a full day in Ohio on Thursday.

Obama's trip is designed to build momentum from two strong debate performances that put his campaign back on a solid footing after Romney bested him in their first debate.

Romney has narrowed the gap on Obama or moved slightly ahead in the eight swing states that will decide which candidate gains the 270 electoral votes needed to win. But in Ohio, a vital part of most victory scenarios for either candidate, a Time magazine poll showed Obama with a 5-point edge on Romney.

In Nevada, which has the highest state unemployment rate at 11.8 percent and has been among the leaders in home foreclosures, Romney warned that four more years of Obama's leadership could be devastating for the economy.

"If he's re-elected, I'm convinced you're going to see the values of your homes continue to bump along in the basement. And you're going to find it hard to get a mortgage as well," he told a rally in Reno, Nevada.

Romney has repeatedly criticized Obama for a slow economic recovery that has left more families in poverty, and the challenger's running mate, Paul Ryan, on Wednesday called for strengthening education efforts and community-based programs to help lift those on the lowest rungs of the economy.

"America's engines of upward mobility aren't working the way they should," Ryan said in a speech at Cleveland State University in Ohio. "There has to be a balance - allowing government to act for the common good, while leaving private groups free to do the work that only they can do."

Billionaire real estate mogul and television personality Donald Trump, a persistent Obama critic who toyed with the idea of seeking the 2012 Republican presidential nomination, offered on Wednesday to give $5 million to the charity of Obama's choice if he released his college and passport records.

Trump, who has questioned whether Obama's birth certificate issued by the state of Hawaii is legitimate, did not say what he expected the records to reveal. However, he asserted in a YouTube video released via his Twitter and Facebook pages that Obama is the "least transparent president in the history of the country."

Office Market Recovery Continues In Third Quarter Despite Election ...

Occupancy, Absorption Levels Hold Serve as Market Indicators Find Recovery Continuing at Slow but Steady Pace

Demand for office space in the U.S. held steady in the third quarter as leasing activity and absorption of available office space continued to pick up momentum following a lackluster start at the beginning of 2012, CoStar Group reported this week in the company's Third-Quarter 2012 Office Review & Outlook.

The overall U.S. office vacancy rate edged down and net absorption rose to 15 million square feet during the quarter from 13 million square feet at mid-year 2012. The relatively little new office supply and continued low levels of new office construction supported the balance in supply and demand.

Meanwhile, office tenants continued to enjoy a 'holiday' from rent increases as office rents in most market have yet to budge much from their market trough tipping point, according to analysts for Property and Portfolio Research (PPR), CoStar?s analytics and forecasting division.

"The recovery is only one-third of the way there in the office sector, we still have two-thirds to go," said Walter Page, PPR director of research, office, who was joined by PPR's Managing Director Hans Nordby and Manager, U.S. Market Research Aaron Jodka, in dissecting the third-quarter numbers for CoStar clients.

If there was a surprise for the quarter it was on the upside. Page said he had expected demand to fall off a bit in the quarter, based on anecdotal reports of companies postponing hiring and expansion decisions due to economic and political uncertainties created by this fall?s hotly contested presidential election and the unresolved budget impasse.

"But it held up, and we expect it to continue to hold," Page said, buoyed by the improvement in job growth over the last couple of months.

CoStar expects leasing activity to exceed 135 million square feet nationally for the third quarter, eclipsing the 130 million square feet of office space leased during the second quarter.

"We've started to see pretty good leasing momentum," Jodka said. "It was a weak first quarter, but we've started to see more decision-making by tenants -- not at the rate we would have expected if it were not an election year or if the economy were performing better -- but we're seeing office-using job growth translate into leasing decisions and momentum , and that's a good sign."

Energy industry driven metros such as Houston, Dallas and Denver, and even previously challenged growth markets such as Atlanta, have posted solid gains in leasing and absorption, both in the third quarter and year to date, fueled by growth in law firms, consulting, insurance and other typical downtown tenant, Jodka noted.

On the other end, consolidation among pharmaceutical companies and other office tenant downsizings have dampened demand in Northern New Jersey. Absorption remains soft in Los Angeles outside the submarkets dominated by media and entertainment companies.

Companies Continue to Pursue Modest Office Hiring

While U.S. GDP slowed to 1.3% in the third quarter from 1.7% in the prior quarter -- hardly the kind of growth arithmetic that induces companies to hire more workers -- office-occupying jobs continued to increase on a year-over-year basis at a faster clip than the national average, Nordby said.

"We're expecting office-using employment to be about 30% better than the overall average in employment. If you believe there's a recovery, then you like the office sector," Nordby said.

Although the U.S. office vacancy rate edged down to 12.6% in the third quarter and is down just half a percentage point from a year ago, PPR continues to expect the vacancy rate to fall another 1.8% through 2016 as supply diminishes and the economy rebounds, by far the steepest drop among commercial property types.

For now, two main factors are constraining office demand. The average amount of office space leased by tenants has shrunk 21% over the last 10 years in a fundamental shift in the way companies use office space due to a variety of factors, including the economic downturn, and the rise of telecommuting and smaller, more collaborative work spaces. Secondly, tenants are still expanding into leased but under-utilized space resulting from layoffs during the recession.

Meanwhile, office tenants that have been active in the market are upgrading their space at the current lower rents. About 70% of net absorption has been in top-tier, 4-Star and 5-Star buildings. However, PPR expects absorption to shift to the middle and lower of the market, similar to the mid-2000s recovery, as the choicest buildings fill up.

Page also pointed out that office demand is improving in metros hurt by the housing crisis, such as Atlanta and Phoenix, with housing clearly less of a drag on their local economies than in previous years. Even in Orange County, where the subprime mortgage collapse caused the office market to crater, mortgage lenders are once again beginning to lease large block of space, Jodka said.

Office Construction Beginning to Stir

Although there?s very little new supply of office space coming on line -- the nation actually removed more space from the inventory than it added in 2011 -- construction of a few new projects totaling 14 million square feet has started in the last two quarters. Most of the new office construction consists of build-to-suit buildings for medical or pharmacautical tenants such as PNC?s 800,000-square-foot headquarters building in Pittsburgh.

However, office construction is becoming a bigger story in a few markets such as San Jose, CA; Boston, Washington, DC, Houston, New York City, Austin and Charlotte, NC. Although up to 85% of the space is preleased in some markets such as Boston, speculative projects have been a higher percentage of total construction in San Jose, DC, Austin and Charlotte.

"We expect that sluggish phenomenon to continue over the next several years until the economy really gets going and profits start to translate into employees and uncertainty starts to wind down," Jodka said.

While the office sector had the greatest increases in vacancy and has not recovered as fast, its recovery probably has more legs going forward because of the lack of construction, Page said.

Stellar duo target Ohio's working class for Obama

(AP) ? Lest anyone forget the importance of Ohio's white, working-class voters, President Barack Obama sent a clear reminder on Thursday.

Make that two reminders: Bill (Clinton) and The Boss (Bruce Springsteen), two aging baby boomers still at the top of their game.

"No retreat, believe me, no surrender," Springsteen sang, performing without the backing of his E Street Band in a darkened gymnasium lit by a spotlight. The lyrics seemed aimed both at the president and his supporters.

With less than three weeks until Election Day, Clinton and Springsteen took the stage to rally support for Obama among the critical middle-class voting bloc in this tightly contested Midwestern swing state.

"For 30 years, I've been writing about the divide between the American dream and the American reality," Springsteen said. "Our vote is the one principal way we get to determine that distance."

Clinton implored voters to reward Obama for bailing out the auto industry, which has deep roots in Ohio.

"When you were down, you were out, and your whole economy was threatened, the president had your back," said the former president and a top surrogate for Obama.

Ohio is at the center of both Obama and Republican Mitt Romney's campaign strategies. Winning the state would put Obama on the brink of the 270 Electoral College votes required to win the White House. Romney, who has fewer pathways to victory than the president, almost certainly needs Ohio's 18 electoral votes if he hopes to claim victory.

Both campaigns are paying special attention to Ohio's working class ? many of whom are white and don't have a college degree. They've made up about half of voters in the state in each of the last two presidential elections.

And they were well-represented among the 3,000 people packed into a community college gymnasium for the Clinton-Springsteen appearance.

"I think Clinton is key," said firefighter Matt Sparling of Parma Heights, Ohio. "He's got an amazing way of keeping it simple." Springsteen, he added, helps draw the crowd.

As Ohio officials opened the event, some in the crowd bellowed "Bruuuuce" ? a standard fan shout-out to the musical legend.

Clinton, too, reveled in the chance to serve as Springsteen's opening act.

"I am qualified because I was born in the U.S.A. and unlike one of the candidates for president, I keep all my money here," Clinton said, referring both to one of the rocker's classic songs and Romney's overseas financial holdings.

Polls show Obama with a lead in Ohio, but Romney has made gains following his strong performance at the first presidential debate.

Each candidate has struggled to connect with white, working-class voters, in Ohio and elsewhere.

Nationally, Romney holds a strong edge among white voters with jobs and no college degree. But his advantage is narrower in union-heavy Ohio. A recent NBC News/Wall Street Journal/Marist poll showed Romney ahead 51 percent to 44 percent among likely noncollege, white voters.

Those heavily-courted voters backed Republicans in the past two elections.

In 2004 in Ohio, 55 percent of white voters without a college education voted for Republican George W. Bush and 44 percent voted for Democrat John Kerry. Four years ago, they voted for Sen. John McCain, R-Ariz., over Obama 54 percent to 44 percent.

This time around, Obama's team sees opportunities to increase its levels of support.

The campaign has blanketed the state with television advertisements portraying Romney as a corporate raider who moved manufacturing jobs overseas. The state's 7.2 percent unemployment rate is more than a half-point lower than the national average. And the auto bailout is credited with saving thousands of jobs in the state.

"I'm thankful GM is still making cars," said Springsteen, whose songs often reflect the stories and values of the people who work in places like auto factories. "What else would I write about?"

In addition to Clinton and Springsteen, Obama is relying on Vice President Joe Biden to rally support among white working-class voters. Biden, raised in a middle-class family from Scranton, Pa., has been a frequent visitor to Ohio. He's scheduled to make a three-day swing through the state next week.

Romney has also relied on his running mate for help in Ohio. Rep. Paul Ryan, a Wisconsin native, has put his Midwestern sensibilities to use, dropping by college football tailgates and a Cleveland Browns practice. He also made a stop at a gun show. Ryan is due back in Ohio on Saturday.

Springsteen, an avid supporter of Obama in 2008, had planned to sit out this election. He never explained exactly what changed his mind in the campaign's closing weeks, saying only that he believes Obama understands the struggles of "everyday citizens."

The rocker kept his comments brief during a six-song set, which included "Youngstown", a song about the economic woes of the Ohio city. Seeking to hew a bit more closely to Obama's re-election message of shared prosperity, he also played a newer song that has become a staple at the president's campaign rallies.

"Wherever this flag is flown," Springsteen sang. "We take care of our own."

You can also search online to find a local climbing partner to create a buddy system for your activity.

Team building sports has always been the favored choice for big corporate brands to increase sales productivity and employee socialization. The direct reason being, when they learn to support each other and themselves through a strenuous challenge, they are more inclined to be able to lead and be a part of a constructive team.

Substance addiction may influence patients' sleep patterns - The ...

The most commonly used substances that affect sleep are caffeine and alcohol. Most patients drink coffee and soda or tea, to some degree, on a daily basis. Alcohol use is also increasing. Many patients will complain of insomnia that may be directly related to their caffeine and alcohol use, without any clue that how this behavior is influencing their sleep. As providers, we have to remember to ask the appropriate questions about substance use when taking a history, including whether our patients are consuming caffeine and alcohol on a regular basis.

The half-life of caffeine is between three and seven hours but patients may experience sleep disturbances up to 10 hours after consuming their last caffeinated beverage. Caffeine is found in coffee, tea, soda, chocolate, stimulant medication, cocoa and over-the-counter analgesics and cold remedies.

Patients who use caffeine can experience insomnia, nervousness, excitability, flushed face and gastrointestinal upset. The effects of caffeine are prolonged in children, pregnant women and the elderly. Patients with hypothyroidism can become toxic with very small doses of caffeine. It may also trigger pain attacks in patients with pain disorders.

Whether caffeine is addictive is questionable; however, ask those who haven't had their morning coffee and they will quickly tell you they can tell a difference when they miss their java. Withdrawal symptoms do appear, including headaches, irritability, fatigue, dysphoria and flu-like symptoms. Patients are known to self-administer if they experience these symptoms, which reinforces the need for caffeine and the development of tolerance. If you recommend discontinuing caffeine, have the patient taper gradually.

Drinking alcoholic beverages close to bedtime can result in decreased total sleep time and decreases in slow-wave sleep. Alcohol does provide a transient sedative effect; therefore, many patients who are anxious or experience insomnia tend to drink prior to bedtime to help fall asleep.

But alcohol also increases snoring and apnea, which are health risks for patients with obstructive sleep apnea. In addition, alcohol use before bed increases periodic limb movements of sleep threefold. Other sleep disorders that increase with alcohol use include parasomnia activity, including sleepwalking and the risk for falling and becoming injured during sleep walking.

The concern for alcohol use and sleep goes beyond the bedroom. Patients who do not sleep well at night because of alcohol use are also more likely to be sleepy during the day. Therefore, driving accidents and workplace injuries can be elevated in this population.

Patients who are true alcoholics may experience sleep issues even one to two years after they stop drinking. Sleep tends to be shortened, shallow and fragmented and the percentage of REM sleep is often elevated. Unfortunately, because of these symptoms, patients often start drinking again. Needless to say, managing these patients' sleep issues is very important during sobriety.

Nicotine is another often overlooked substance that affects sleep. However, smokers often have a difficult time falling asleep with increased arousals throughout the night. A new symptom has been described in established smokers called nocturnal sleep-disturbing nicotine craving. The prevalence in approximately 20% and is characterized by patients who wake up one or more times during the night and have to smoke before they can return to sleep.

Remember the importance of asking your patients about any substances they may be, using including illegal substances, to educate them about the health risks -- not only to help improve sleep, but to encourage general well-being. Besides managing sleep issues, we also have to address the potential addictions patients may be experiencing, and refer them to the proper counselors who may be able to help.

Oil price ends higher amid mixed economic data

(AP) ? The price of oil finished slightly higher Tuesday, following a day of wavering between small gains and losses as traders mulled mixed economic data.

The Labor Department said the consumer price index rose 0.6 percent last month, the same as in August, with inflation still in check.

Meanwhile the Federal Reserve said industrial production was barely higher last month, and factory output showed a decline in the July-September quarter. Weak activity in the nation's manufacturing sector generally means less demand for oil.

Benchmark U.S. crude ended up 24 cents at $92.09 a barrel in New York.

Brent crude, which is used to price international varieties of oil, lost 40 cents to finish at $114 a barrel in London.

At the pump, the national average for a gallon of gas fell a little more than a cent from Monday, to $3.77.

Other futures on the New York Mercantile Exchange:

? Heating oil fell a penny to end at $3.20 a gallon.

? Natural gas lost 5 cents to finish at $3.44 per 1,000 cubic feet.

? Wholesale gasoline ended down a half-cent at $2.85 a gallon.

At CDC, scientists fight to halt a deadly outbreak

ATLANTA (AP) ? Scattered across the carefully landscaped main campus of the Centers for Disease Control and Prevention are the staff on the front lines fighting a rare outbreak of fungal meningitis: A scientist in a white lab coat peers through a microscope at fungi on a glass slide. In another room, another researcher uses what looks like a long, pointed eye dropper to suck up DNA samples that will be tested for the suspect fungus.

Not far away in another building is the emergency operations center, which is essentially the war room. There's a low hum of voices as employees work the phones, talking to health officials, doctors and patients who received potentially contaminated pain injections believed to be at the root of the outbreak. Workers sit at rows of computers, gathering data, advising doctors and reaching out to thousands of people who may have been exposed. Overall, dozens of people are working day and night to bring the outbreak under control. More than 200 people in 14 states have been sickened, including 15 who have died.

There is a sense of urgency ? people are dying, and lives could be saved if those who are sickened get treated in time. But it's not a race against a fast-spreading illness like avian flu or SARS ? or even the fictional virus the CDC fails to unravel in the popular TV series "The Walking Dead." Unlike those outbreaks, this strain of meningitis isn't contagious and doesn't spread between people. It is likely isolated to the contaminated steroid, produced by the New England Compounding Center in Framingham, Mass.

"This is a very unusual infection," said Dr. John Jernigan, a CDC medical epidemiologist who is leading the clinical investigation team for the outbreak response. "So, treatment recommendations, diagnostic recommendations are all going to be new, and we're learning as we go on this one."

Meningitis, an inflammation of the membranes surrounding the brain and spinal cord, is not uncommon. But it is usually caused by bacteria, and it is very unusual to see it in patients with normal immune systems, Jernigan said. This strain is caused by a fungus that is common in dirt and grasses ? people routinely come into contact with it without getting sick ? but it has never before been identified as the cause of meningitis.

By Friday morning, officials believed they had reached about 90 percent of those who were potentially affected, Jernigan said. They planned to continue trying to reach every person to see if they've had problems and to warn them to be on the lookout for symptoms, which can include severe headache, nausea, dizziness and fever. The CDC says many of the cases have been mild, but some people had strokes.

A meeting is held each morning to review overnight developments and plot a course of action for the day, and another at the end of the day summarizes the day's developments and looks ahead to the next day. Maps on big screens in the front of the emergency operations center track the states where the tainted medications were sent and the tally of cases reported in affected states.

A few steps away in the joint information center, another team works to keep the information about the outbreak on the CDC's website up to date and disseminate information via the media and other outlets.

In another building on the campus tucked away in the northeast corner of Atlanta, in a part of the CDC that specializes in fungal infections, about 15 scientists in the reference and research labs are logging 12 hours or more a day and working through weekends to test samples coming in from around the country.

Because the lab scientists had never worked with this particular fungus in cerebrospinal fluid before, they had to quickly develop new tests to detect it before they could start analyzing the hundreds of samples ? cerebrospinal fluid samples, cultures and bits of tissue ? sent in from around the country, research lab team leader Ana Litvintseva said.

Dressed in a white coat Friday, Shawn Lockhart, the fungal reference lab team leader, peered through a microscope as images of what looked like red pea pods appeared on a computer screen next to him. Many other closely related fungi look similar, but a tiny dot at the end of a pod told him he was looking at the fungus believed to be at the root of the outbreak.

Normally, the reference lab works on difficult samples sent in from state health departments, while the research lab works on research projects. But the scale of this outbreak means those projects are mostly being shelved at the moment.

"The scale is much, much bigger than we would normally work with," said research lab team leader Ana Litvintseva said. "We are working every weekend and people are here 12 to 13 hours at a time and we're testing samples nonstop."
